Troop 11 Medical Form Instructions Summer Camp
Three (3) hardcopies of Parts A, B1 and B2 are required to be submitted in September (by September 30) each year. This applies to all Scouts, Scoutmasters, and any parents who plan to attend any trip, including Summer Camp, during the Scouting year.
Part C is not required in September. Part C will be collected as needed for trips over 72 long. Note that Part C is required for all Scouts and adults attending Summer Camp.
Front and back copies of the insurance card must be included with each copy of the medical form. *** TIP *** *** TIP *** *** TIP *** *** TIP *** Download the BSA Annual Health and Medical Record form and save it to your computer.
To minimize the amount of effort required to fill out the form every year, fill out the form on your computer and save it. Each year you’ll only need to update the form with any changes, print it, then sign and date it with the new date.
The following instructions include commonly missed items on the form. Please make sure these items are complete before you turn in the 3 copies of the form.
Part A
1. If you do not want your child using a BB device, check the box to indicate that.
2. If there are any restrictions Scoutmasters should be aware of, list them. If none, check the box for None.
3. You must list at least one adult authorized to pick up your Scout.
4. If you are filling out the form online, DO NOT sign the form digitally or you won’t be able to use it in subsequent years. Print out the form and sign and date it by hand.
Part B1
1. If you are updating the form from a previous year, update the Age, Height, and Weight fields. 2. Unit leader: Kim Brennan

6. Every box in the Health History section must be checked ‘Yes’ or ‘No’.
Part B2
NOTE: To ensure everyone’s health and safety, the lead Scoutmaster on each trip is notified of any Scouts with allergies or medications. If you do not want this information communicated, please let the Medical Form Coordinator know.
1. Check all boxes ‘Yes’ or ‘No’ in the Allergies/Medications section.
2. Enter all medications, or check the box if no medications are routinely taken.
3. Check ‘Yes’ or ‘No’ to authorize Scoutmasters and chaperones to give any over-the-counter medicine as needed. This also applies to the Camp Nurse at Summer Camp.
4. If you are filling out the form online, DO NOT sign the form digitally or you won’t be able to use it in subsequent years. Print out the form and sign and date it by hand. An MD signature is not required.
5. Immunizations must be listed with the last date received.
6. Tetanus can be listed as either Tdap or DTaandnd must have been administered within the past 10 years.
